Should I add in the complaint regarding child support . prays the court award child support as determined by the Work Sheet B?

Also When filling out the Affadavit for Status of the Child. Can my parents sign it. I have several people that know that I keep my son, but they do not know where my wife lives so they can only attest to my keeping him. They can say they know she keeps him, but do not know wheer she lives, so I am thinking they can only attest to me. How important is it that they know her address, or even mention her?

You do not need to ask the court for a worksheet B, the court will make findings of fact regarding the amount of overnights the child(ren) are with the non-custodial parent, and will use the appropriate worksheet.

You need to sign the affidavit of status, not your relatives.
