Completing the Civil Summons form

I am trying to complete the civil summons form and it has a part to write in the date issued, what date is this? is this the date that I filed? or what date?

The “date issued” box will be filled out by the clerk when you go to file your complaint. The clerk will issue the civil summons. You do not need to complete that section.

The only sections of the civil summons that you need to have filled out are the name and address of the plaintiff (you), name and address of the defendant, and to the left of the “date issued” box you need to put your name and address again.
