How do you address a judge in letter?

I have to write a letter to the judge about the orders she stipulated at the temp hearing not being written correctly. The opposing attorney changed the facts of findings and added some things that the judge did not say.

Do I write “Dear Judge So/So”? Or do you use “To the Honorable Judge So/So”?
