Your ex would need to refinance the loan into a new loan or loans in his sole and separate name in order to remove you from liability on the note. To remove your name from the title you will need to execute a Deed transferring your ownership interest in the home to him. I do not advise you execute a deed until he can prove he has refinanced the debt on the home.
