I’ve been told I need to file a motion to show cause so that I can get in front of a judge to have day care after school stopped for my almost 13 year olds.

Is this the correct motion? Is their a court form? If so what is the number?

I would also like to summons the daycare. What form do I use for this? Do I file the summons the same time as the motin to show cause?
