I have to file discovery info tomorrow morning in Durham. Please tell me what form to use and any other useful info. Ty

There is no form for discovery. Discovery can be different types of documents, for example, interrogatories (written questions to the other party), requests for production of documents and things (a written list of documents that one party needs from the other), request for admissions (a list of statements that the other party has to admit or deny), etc. The party filing the discovery requests drafts their own document and serves it on the other party.
