You need to file a motion for alimony and ask the court for a hearing on post separation support. During that hearing you can ask that your health insurance be continued.

what form and what is the number of it do i use to file this motion ?

There is no form to file for alimony, you would need to file a complaint which is the legal document that sets forth your claim for alimony.

my wife and i are not ledally seperated

we both had ex partes agianst each other. at first she was removed from the home. i dropped my ex parte and she continued hers.
the judge let her return to the home and made me leave.

i am disabled and she is threataning to stop my health insurance.

can i file some kind of injunction to stop her from doing so and what documents do i use.
she has been my primary source of income i have no money for an attoreny
