Modification of Child Support from 2012

In 2012, there was a child support order established and my husbands ex wife at the time was not working, but was going to school for her bachelors degree… She has since obtained a Masters Degree, is working for the state and occasionally at 2 other places… We are planning to file the modification papers ourselves. Can you provide a list of the forms necessary to file? I have WAKE-DOM-02, AOC-CV-600, Rev. 3/03, AOC-CV-640, AOC-CV-645. Are those the only forms we would need or is there some that I am missing? Since they aren’t really in a specified order on NC Courts, I am struggling to determine which papers we need to file! Any help would be great.
