My daughter has moved to another state..i want joint custody

HELLO EVERYBODY. FIRST OFF,I AM IN THE ARMY I AM A SINGLE FATHER OF A SON WHO IS 7YRS OLD. HIS MOTHER LEFT WHEN HE WAS 1 AND HAS NOT RETURNED, BUT I HAVE A LOVING WIFE THAT HAS TAKEN HIM IN AS HER OWN. MY QUESTION IS MY ABOUT MY 5 YR OLD DAUGHTER WHO NOW RESIDES WITH HER MOM IN CHARLOTTE NC I RESIDE IN AUGUSTA GA. SHE TOOK ME TO COURT WHILE SHE LIVED IN TENNESSEE FOR CHILD SUPPORT AND GOT PRIMARY CUSTODY AND WE KEPT THE VISITATION OPEN BECAUSE I AM IN THE ARMY AND I THOUGHT WE WOULD BE ABLE TO HANDLE THINGS LIKE ADULTS. HOWEVER SHE IS NOT ALLOWING ME TO SEE MY DAUGHTER BUT RECIEVES 500 A MONTH IN CHILD SUPPORT AND COVER MEDICAL/DENTAL AND VISION. WE R SUPPOSE TO SPLIT THE COSTS OF MEDICAL BUT THE BILLS KEEP COMING TO MY HOUSE AND SHE WONT HELP PAY THEM.I WANT JOINT LEGAL AND PHYSICAL CUSTODY OF MY DAUGHTER, I CANNOT AFFORD AN ATTORNEY. CAN YOU LET ME KNOW WHAT FORMS I NEED TO FILL OUT TO GET THE BALL ROLLIN. THE CURRENT ORDER IS FROM ANOTHER STATE 2 YRS AGO. THE MOTHER AND MY DAUGHTER NOW RESIDE IN NC. PLEASE HELP… I WANT TO SEE MY DAUGHTER

You will need to file a motion in the state that issued the current order to have them release jurisdiction to North Carolina.

Once that is done you will need to file a motion to modify custody and child support based on a substantial change in circumstances (your not being allowed to see your child).

There is no form per se for such a motion, however Mecklenburg County has a self service clinic which you may find useful.
