You can only kick her off the insurance if it is an open enrollment period or if you are divorced. If you have been separated for a year already you can file for divorce and when it is granted she will be kicked off your insurance.

My “Wife” is pregnant to another man. We have ben seperated for more than a year and she is still on my insurance. She got pregnant after the 12+ months of seperation.

Can I kick her off the insurance even if we are not divorced?
Do I have any legal action towards her? or the other guy?

Tip for all the nice guys out there: Kick her to the curb after she does that to you. You have no obligation to still be a “man” towards her.
