Acceptance of Service Form

I asked last week about how to bypass the Sherriff and Certified Mail options if my wife is willing to sign a form accepting the civil summons (she works out of town and it would be a pain otherwise). The attorney said I could use the “Acceptance of Service” form on your web site. The problem is I can not find that form anywhere on your site (or anywhere else for that matter). Could you point me in the right direction? That form is the only item I need to go fully prepared to the Wake Co. Court House to file.

BTW, is the $144.00 amount to file sill accurate?

The form is available on our DIY site, to which you must have a subscription. I believe the filing fee for absolute divorce is now $168.00.
