I have 3 children and am filing a motion to modify custody.
I have to file a Affidavit as to Status of Minor Child (AOC-CV-609).
Can I put all 3 kids on one form, or do I need one form per child ?

The form only lets you put in information for one child. You’ll have to do one for each.
