If she legally changed her name when you married, then list only her married name. If she did not legally change her name, then you should list both.

Hello. My wife is a resident alien and her maiden name is listed on our marriage certificate, her foreign passport and her US Visa, while her married name is listed on most post-marriage documents. Should her maiden name be listed as an AKA on our court divorce documents and, if so, does the AKA go on each form?
