Filing multiple contempt motions simultaneously

I need to file several contempt motions and want to combine them so they can be heard at the same time. There are basically three issues where my ex-spouse is out of compliance with two different court orders. I assume that I would fill out the contempt form detailing the violations and orders, and then attach the orders themselves to the motion. Is this correct? Is there anything else I would need to include in the filing, such as supporting evidence specific claims?

You may attach exhibits to your motions if you wish, but actual evidence should be presented to the court at the hearing on the contempt motion.
