I am representing myself, I am completing the form for a Supoena…the space requesting “Name and Location of Court/ Place of Deposition/ Place to Produce” …do I put the Clerk of Court address? or just Court address? Or my address? Also, do I sign in the space for Signature or is that reserved for the Clerk of Court?
If you are requesting documents you may put your address. Leave the space blank for the clerk to sign it.
