Call your dad and let him know and just move in with him. You are old enough to decide who you want to live with.

Make sure that you have valid reasons for wanting to make this change, in case this does have to go to court. The judge may want to know why and it can’t be petty things like, mom makes me carry out the trash and I don’t want to do that anymore. But Phil is right, you are old enough to decide and should be able to express your reasons to both your parents. Let them know and see what happens next.
