Civil Summons Question

I have found the PDF of the civil summons form on your website. However, the directions say that you will get one white copy and two yellow copies of the summons.

So is this a document that I can fill out ahead of time and take with me to file (if so will they make the yellow copies?) or do I obtain it at the courthouse and fill it out there?

You will need to fill out the party information (names and addresses) and take the two yellow copies and one white copy to the clerk so they may fill out the portion of the summons to issue the same.
